Full-Time | Direct Hire | Onsite | Day Shift | Monday–Friday, No Weekend Rotation

Key Responsibilities

• Evaluate and treat patients in a skilled nursing and short-term rehabilitation setting
• Develop and implement individualized physical therapy treatment plans
• Monitor and document patient progress toward established goals
• Collaborate with interdisciplinary care team to coordinate patient care
• Provide education to patients and families regarding treatment and home exercise programs

Required Qualifications

• Active Physical Therapist (PT) license in the state of West Virginia
• Graduate of an accredited Physical Therapy program
• Ability to work 40 hours per week, five days per week

Preferred Experience / Skills

• Prior experience in a skilled nursing or long-term care environment
• Experience with short-term rehabilitation patient populations
• Strong documentation and communication skills
