Q: When am I eligible to enroll in benefits?
A: You are eligible to enroll on your first day of employment. You have 30 days to complete your enrollment.
Q: When do my benefits begin?
A: Medical, dental, and vision begin 7 days after you enroll. All other benefits begin the first of the month following 30 days of employment.
Q: What happens if I miss my 30-day enrollment window?
A: You’ll need to wait until the next open enrollment period or experience a qualifying life event to make changes or
enroll.
Q: What is a qualifying life event?
A: Examples include marriage, divorce, the birth or adoption of a child, or loss of other coverage. You must enroll as a life event within 30 days of the event to make benefit changes.
Q: What kind of medical plan is offered?
A: A High Deductible Health Plan (HDHP) through Anthem. It includes 100% coverage for in-network preventive care, prescription benefits, and the option to use a Health Savings Account (HSA).
Q: Are dependents eligible for coverage?
A: Yes. Spouses and dependent children, until age 26, can be covered under all available benefit options.
Q: When does my coverage end if I leave the company?
A: Coverage ends the Sunday after your last day worked.
Q: Can I continue coverage after leaving the company?
A: Yes. You will be eligible for COBRA continuation coverage. You should receive information within 3–5 business days after termination, and there will be no lapse in coverage if you enroll.
Q: What is the 401(k) match?
A: After one year of service, Malone matches 100% of the first 3% you contribute, plus 50% of the next 2%. Entry dates are January 1 and July 1.
Q: What happens if I miss a check for vacation, RTO, or being out sick?
A: You will be double deducted before your missed checks or when you return, but all missed premiums must be paid.
Q: What happens if I end my contract but would like to keep my insurance?
A: The only way to keep insurance active is to have a new signed contract. Your payments for the coverage will be
deducted before your current contract ends or after you start your new contract.
Q: When will I receive my insurance cards?
A: Your insurance cards will be mailed within two weeks to the address you originally provided. If you would like your cards mailed to a different address, please email hr@malonesolutions.com.
Q: When are digital insurance cards available?
A: Digital cards are available within 48 hours of your effective date. Please reach out to hr@malonesolutions.com to obtain a copy of your card.
Q: Is a contract extension considered a qualifying life event?
A: Yes, if you did not enroll at your original start date you can enroll when you start your extension.
