Key Responsibilities
- Evaluate and treat patients in accordance with individualized care plans
- Develop and implement physical therapy treatment programs to restore function and improve mobility
- Document patient progress and maintain accurate clinical records
- Collaborate with interdisciplinary care team to support patient goals
Required Qualifications
- Active Physical Therapist (PT) license in the state of California
- Graduate of an accredited Physical Therapy program
- Expected 35 hours per week
Preferred Experience / Skills
- Prior experience in a skilled nursing facility or long-term care setting
- Experience with geriatric patient populations
