Full-Time | Permanent | Onsite | Days | Monday–Friday, 8-hour shifts (4×10 schedule may be available) | Weekend rotation approximately every 6–7 weekends | Rare holiday rotation

Key Responsibilities

• Provide physical therapy services primarily in an outpatient setting, with some inpatient coverage and occasional SNF and home health patient care
• Evaluate, assess, and develop individualized treatment plans for a diverse patient population
• Document patient progress and maintain accurate records using the Epic electronic medical records system
• Collaborate with an interdisciplinary care team to deliver high-quality, community-focused rehabilitation services
• Participate in weekend and holiday rotation schedules as assigned

Required Qualifications

• Active Minnesota Physical Therapist license required
• Basic Life Support (BLS) certification required within one year of hire
• Minimum of 1 year of experience in a health-related field
• Proficiency with electronic medical records systems; Epic experience preferred

Preferred Experience / Skills

• Experience working across multiple care settings including outpatient, inpatient, SNF, and home health
• Familiarity with Epic charting system
• Strong interpersonal and communication skills with a commitment to team-based, community-centered care
