Key Responsibilities

• Evaluates and treats patients of all ages, modifying the plan of care as appropriate based on patient response and available evidence
• Performs patient examinations using reliable and valid methods relevant to the chief complaint, screening results, and history; determines physical therapy diagnoses and outcomes of care
• Designs individualized plans of care integrating goals, treatment, outcomes, and discharge planning, including objective testing for range of motion, strength, and functional tasks
• Completes daily documentation and charging in therapy EMR in accordance with clinical process policies
• Applies therapeutic modalities such as ultrasound, interferential, and iontophoresis, as well as taping techniques for stabilization, facilitation, or inhibition of movement
• Communicates with referring providers on patient progress and treatment recommendations; educates patients, families, caregivers, and other healthcare providers using effective teaching methods

Required Qualifications

• At least a Bachelor’s degree in Physical Therapy
• Active Illinois Physical Therapist (PT) License
• BLS certification through the American Heart Association (AHA)
• Minimum of one (1) year of relevant physical therapy experience
• Knowledge of safe body mechanics and ability to perform patient transfers as needed

Preferred Experience / Skills

• Experience evaluating and treating patients with orthopedic and neurologic conditions
• Familiarity with supervising and monitoring support personnel in accordance with legal and ethical standards
