Schedule / Job Type

Contract | Full-Time | Days | Monday–Friday, No Weekends | Variable start time (7:30 AM or 8:00 AM) | 8-hour shifts | No on-call requirements

Key Responsibilities

• Complete evaluations to ensure quality standards and address assessment areas requiring clinical attention
• Develop and implement individualized treatment plans using appropriate clinical techniques, tools, and treatment protocols
• Establish individualized care and discharge plans and communicate them to appropriate care team members
• Complete comprehensive and accurate clinical documentation within established deadlines
• Serve as co-leader in an interdisciplinary approach to patient care groups
• Supervise assigned therapy assistants as applicable

Required Qualifications

• Active Physical Therapy licensure in the state of practice required
• CPR or Basic Life Support (BLS) certification from an approved vendor required
• Doctor of Physical Therapy (DPT) degree required if license was obtained on or after January 2016, or foreign equivalent
• Minimum of 2 years of recent Physical Therapy experience
• EPIC electronic health records experience required

Preferred Experience / Skills

• Experience working in an acute care or hospital-based setting
• Familiarity with interdisciplinary care team collaboration and patient group treatment models
• Experience with advanced wound care or rehabilitation program environments
