Contract (17 Weeks) | Full-Time | Days | Monday – Friday | Onsite
Key Responsibilities
- Evaluate and treat patients in accordance with individualized physical therapy plans of care
- Document patient progress, treatment sessions, and clinical outcomes in a timely manner
- Collaborate with interdisciplinary care team to support patient rehabilitation goals
- Maintain a guaranteed 40-hour work week as outlined in contract terms
- Must carry and present a valid state or federal issued photo identification at the facility at all times
Required Qualifications
- Active Physical Therapist (PT) license in the state of New Hampshire
- Graduate of an accredited Physical Therapy program
- Valid state or federal issued photo identification required on-site at all times
Preferred Experience / Skills
- Prior experience in a skilled nursing or long-term care setting
- Experience with geriatric patient populations
- Strong documentation and communication skills
