Key Responsibilities

• Provide occupational therapy services in a home health setting, including evaluations and revisits
• Complete an average of four patient visits per day across an assigned coverage area
• Deliver care across a service area spanning the north-eastern Sacramento region into Folsom and the Woodland/Yolo County area
• Maintain accurate and timely clinical documentation in accordance with home health standards
• Coordinate care with the broader clinical team to support patient goals and outcomes

Required Qualifications

• Active California Occupational Therapist (OT) license required
• BLS certification required
• Active NPI number required
• Minimum of 2 years of experience in a healthcare setting
• Valid driver’s license and proof of current automobile insurance required

Preferred Experience / Skills

• Prior home health experience strongly preferred
• Candidates holding a COTA license with home health experience may be considered
• Familiarity with evaluation and revisit workflows in a community-based care setting
