Contract | Days | Monday–Friday, 8:00 AM–5:00 PM | No weekends or holidays

Key Responsibilities

• Conduct home health visits including start of care, recertifications, resumption of care, and regular patient visits
• Expected caseload of approximately 6.5 points per day; typically 5–6 regular visits per day, or 3 visits on admission days
• Perform occasional safety evaluations or one-time teaching visits in hospice situations as needed
• Maintain accurate and timely clinical documentation in accordance with home health standards
• Coordinate with clinical leadership and peer therapists at sister agencies while working independently in the field
• Adhere to dress code: black, navy blue, or grey scrubs

Required Qualifications

• Active North Dakota Physical Therapy License
• Current BLS Certification
• Valid Driver’s License and current Auto Insurance required
• Must live at least 50 miles from the facility to be eligible
• Candidates who have been employed full-time, part-time, or PRN with any CommonSpirit, CHI, or Dignity-affiliated facility within the past 12 months are not eligible

Preferred Experience / Skills

• Prior home health experience strongly preferred
• OASIS training and experience preferred
• Comfort working independently in a field-based environment with remote clinical support
