Job Title

Registered Nurse (RN) – Home Health

Contract | Days | Five 8-hour shifts per week | Weekend and holiday rotation required

Key Responsibilities

• Manage a caseload of approximately 30 adult patients in a home health setting
• Complete Start of Care (SOC) assessments, recertifications, and daily visits, targeting 6.5 points per day
• Provide skilled nursing care and clinical assessments during in-home visits
• Participate in weekend rotation based on staffing levels (hours: 8am–5pm)
• Participate in holiday rotation as required
• Utilize agency-provided phone and tablet for documentation and communication while in the field

Required Qualifications

• Active RN license in the state of Kentucky — must be in hand at time of submission
• Home Health experience required — no exceptions
• Home Care Home Base (HCHB) experience required — no exceptions
• Minimum 1 year of experience on a Med/Surg floor in a hospital setting
• Experience performing Start of Care assessments, recertifications, and routine home visits

Preferred Experience / Skills

• Familiarity with adult-only home health patient populations
• Strong time management skills with ability to meet daily productivity point requirements
• Comfortable working independently in the field with access to remote clinical support
