Case Manager – Registered Nurse (Nursing)

Mattoon, IL | Acute Care
Schedule / Job Type
Contract | Full-Time | Days | Monday–Friday, 8:00 AM–4:30 PM | No weekends, major holidays, or call required
Key Responsibilities
- Provide case management services across all inpatient units
- Manage care coordination for ED holds and overflow units
- Assess, plan, and facilitate patient care to support appropriate utilization of resources
- Collaborate with interdisciplinary teams to coordinate discharge planning and transitions of care
Required Qualifications
- Active Registered Nurse (RN) license required
- Minimum of 5 years of experience as a Registered Nurse
- Minimum of 3 years of case management experience
- In-hospital (inpatient) nursing experience required
Preferred Experience / Skills
- Experience working across multiple inpatient units or service lines
- Familiarity with utilization review and discharge planning processes
- Case Management certification (e.g., CCM or ACM) is a plus
