Key Responsibilities
- Provide direct patient care in a long-term care / skilled nursing facility setting
- Administer medications and treatments as directed by the care plan and supervising clinicians
- Monitor and document patient conditions, reporting changes to appropriate clinical staff
- Collaborate with interdisciplinary team members to support resident health and safety
- Maintain valid state LPN/LVN licensure and adhere to all facility compliance requirements at all times
Required Qualifications
- Active and unrestricted LPN or LVN license in the state of New Mexico
- Current CPR/BLS certification
- Must present a valid state or federal issued photo ID and wear a photo badge at all times while on assignment
- Must have completed the original or updated COVID-19 vaccine series, or be able to present a documented religious and/or medical exemption from your employer
Preferred Experience / Skills
- Prior experience in a long-term care or skilled nursing facility environment
- Familiarity with electronic health records (EHR) and clinical documentation systems
- Strong communication and teamwork skills within a multidisciplinary care setting
