Registered Nurse – Home Health

Contract | Days | 5×8 Hours (8:30 AM – 5:00 PM) | Onsite
Key Responsibilities
- Provide skilled home health nursing visits to patients primarily transitioning from hospital or nursing facility settings
- Perform and manage complex procedures including wound care, wound vacs, Pleurx drains, and ostomies
- Care for patients with diagnoses including Heart Failure, Diabetes, oncology, post-surgical, and orthopedic conditions
- Complete OASIS assessments and document all care accurately using EPIC EMR
- Maintain a productivity standard of 6 visit-equivalents per day (e.g., admissions count as 2 visits)
- Rotate every 5th weekend and every other holiday as part of the standard schedule
Required Qualifications
- Active New Jersey RN License required; must be in hand at time of submission — pending licenses are not accepted
- Minimum 2 years of recent Home Health experience, with experience within the last 12 months
- Strong Med-Surg clinical skills
- Proficiency with OASIS and EPIC EMR
- Valid driver’s license and reliable personal transportation required
- Active BLS certification required; must not expire within 30 days of assignment start date
- Candidates living within 50 miles of the primary facility are subject to the local rate
Preferred Experience / Skills
- Experience caring for oncology, orthopedic, or post-surgical home health patients
- Familiarity with Heart Failure and Diabetic patient management in a home setting
- Experience with Pleurx drains and advanced wound care modalities
