Registered Nurse – Case Management (Hospital)

Contract | Days | Monday–Friday | 5×8-Hour Shifts (8:00 AM – 4:30 PM) | No Weekends | No Call
Key Responsibilities
- Assess, evaluate, and drive individualized discharge planning for each patient
- Coordinate services to transition patients to the appropriate level of care
- Initiate, evaluate, and re-evaluate treatment plans on a daily basis
- Monitor length of stay and proactively identify and address barriers to discharge
- Follow patient progress daily and adjust care coordination strategies accordingly
- Adhere to Medicare regulatory guidelines regarding patient choice, IMM notices, and MOON requirements
Required Qualifications
- Active Georgia RN license or valid Compact RN license required at time of submission
- Minimum of 3 years of case management experience required; first-time travelers will not be considered
- Strong knowledge of discharge planning, care coordination, and utilization management
- Ability to complete all compliance documentation by the Tuesday prior to start date
- All requested time off (maximum 7 days per contract) must be finalized at time of submission; no schedule changes or RTO requests will be accepted post-offer or on-site
- Local candidates are welcome and accepted
Preferred Experience / Skills
- Prior travel nursing experience in a hospital case management setting
- Familiarity with Medicare/Medicaid regulatory requirements and compliance standards
- Experience working in a fast-paced acute care environment with high patient volumes
