Registered Nurse – Home Health

Contract | 13 Weeks | Days | 8–10 Hour Shifts | Onsite | Expected 36 hours/week
Key Responsibilities
- Conduct an average of 5 patient visits per day in the field, with potential for up to 6 visits depending on territory, patient census, and staff availability
- Perform OASIS admissions, all time points, and episode management
- Coordinate care including physician communication and documentation in EMR in a timely manner
- Demonstrate clinical competencies in wound care, IV therapy, port management, lab draws, and chronic disease management
- Participate in every 3rd weekend and holiday rotation, with on-call responsibility every 4th weekend and one day per week
- Adhere to all facility health and safety policies, including masking requirements for staff who decline applicable vaccinations during flu season
Required Qualifications
- Active Registered Nurse (RN) license in both Michigan and Wisconsin required
- AHA BLS certification required
- Valid Driver’s License required
- OASIS charting experience required
- Ability to travel up to 150 miles in a given day as needed based on territory and patient needs
Preferred Experience / Skills
- 2+ years of home health nursing experience preferred
- Epic EMR experience preferred
- Strong time management, adaptability, and ability to manage change in a dynamic environment
- Compassionate, dependable, accountable, and team-oriented with a positive attitude
