Key Responsibilities
- Provide direct patient care and assistance with activities of daily living in a long-term care / skilled nursing facility setting
- Monitor and report changes in patient condition to nursing staff
- Assist with repositioning, transfers, and mobility support as needed
- Maintain accurate documentation of patient care activities in accordance with facility standards
- Adhere to infection control protocols and facility safety policies at all times
- Must carry and present a state or federally issued photo ID at the facility and wear a photo badge at all times while on assignment
Required Qualifications
- Active CNA certification or T-CNA status in the state of New Mexico
- Long-term care or skilled nursing facility experience
- Ability to work night shifts on Tuesday, Wednesday, and Thursday
- COVID-19 vaccination required; documented religious or medical exemption from employer may be accepted in lieu of vaccination
- Any additional vaccine doses must be obtained where required by applicable law
Preferred Experience / Skills
- Prior experience in a long-term care or SNF environment
- Familiarity with electronic health records and care documentation systems
- Strong communication and teamwork skills in a fast-paced care setting
