Contract | Days | Weekend rotations expected once every 6 weeks
Key Responsibilities
- Provide physical therapy services to patients in their place of residence who have physical impairments, functional limitations, or disabilities resulting from injury, disease, or other causes
- Serve a patient population ranging in age from birth to over 100 years
- Collaborate with patients, families, physicians, and other allied health professionals to identify patient needs and coordinate services accordingly
- Maintain thorough and timely electronic documentation in accordance with agency policies, procedures, and standards of practice
- Communicate effectively across interdisciplinary teams to support coordinated patient care
- Travel to patient cases as assigned within the service area
Required Qualifications
- Active Physical Therapist license in the state of Massachusetts required
- Minimum of 1 year of home care experience required
- Ability to travel to assigned patient cases
- Availability for weekend rotations approximately once every 6 weeks
Preferred Experience / Skills
- Experience working with a diverse patient population across a wide age range
- Familiarity with electronic medical record (EMR) documentation systems in a home health setting
- Strong interdisciplinary communication and care coordination skills
