RN Float – Home Health

Contract | Days | Weekend rotations expected once every 6 weeks
Key Responsibilities
- Assess patient conditions and implement individualized care plans in alignment with organizational philosophy, policy, and community health practice standards
- Provide ongoing education and support to patients and their families throughout the care process
- Perform admission processes and fulfill case management responsibilities in accordance with established policies and standards of practice
- Maintain thorough and timely electronic documentation and facilitate interdisciplinary communications
- Travel to assigned patient cases within the service area
Required Qualifications
- Minimum 1 year of experience within a Certified Home Health Agency — experience in hospice, private care, or general case management alone does not satisfy this requirement
- Minimum 1 year of Case Management experience
- Active Registered Nurse (RN) licensure
- Ability to travel to cases as assigned
Preferred Experience / Skills
- Familiarity with electronic health record (EHR) documentation systems
- Strong interdisciplinary communication and collaboration skills
- Experience managing a diverse patient caseload in a community health setting
