LPN – Full Time Days/Evenings (Long-Term Care)

Key Responsibilities
- Collect and communicate data relevant to residents’ physical and psychosocial condition on an ongoing basis to the RN and Interdisciplinary Team.
- Contribute to the development of age-appropriate and culturally relevant care plans reflecting nursing diagnoses and interventions, involving residents, families, and the healthcare team.
- Evaluate resident outcomes and recommend revisions to the plan of care; identify needs for referrals to ensure comprehensive care is addressed.
- Provide direct and indirect skilled nursing care for assigned residents under the supervision of a Registered Nurse, following defined policies and procedures.
- Maintain collaborative relationships with nursing staff, physicians, and ancillary staff while promoting a safe and healthy environment.
- Participate in staff mentoring and orientation; maintain current knowledge and competency in nursing practice through continuing education and in-service programs.
Required Qualifications
- Active New York State LPN Licensure
- BLS Certification through the American Heart Association (AHA) or RQI specifically required
- Minimum 1 year of experience as an LPN
- Ability to provide care to adult and geriatric patient populations, with demonstrated knowledge of growth and development across the life span
- Availability for every-other-weekend and holiday shifts as required
- Ability to begin orientation on a Tuesday or Thursday for a full-shift orientation day
Preferred Experience / Skills
- Prior experience in a long-term care or skilled nursing facility setting
- Familiarity with interdisciplinary team collaboration and resident/family-centered care models
- Experience mentoring or orienting new nursing staff
