Job Title
Occupational Therapist (OT) / Occupational Therapy Assistant (OTA)

Key Responsibilities
- Evaluate and treat patients in a skilled nursing facility setting to improve functional independence
- Develop and implement individualized occupational therapy treatment plans based on patient goals and clinical findings
- Document patient progress, treatment plans, and outcomes in accordance with facility and regulatory standards
- Collaborate with interdisciplinary team members to coordinate patient care
- Provide patient and caregiver education to support carry-over of therapeutic activities
Required Qualifications
- Active California OT or COTA license in good standing
- Graduate of an accredited Occupational Therapy or Occupational Therapy Assistant program
- Current CPR/BLS certification
- Experience working in a skilled nursing or long-term care environment
Preferred Experience / Skills
- Prior experience in a SNF or post-acute rehabilitation setting
- Familiarity with MDS documentation and Medicare compliance requirements
- Strong communication and team collaboration skills
