Key Responsibilities
- Provide direct patient care in a long-term care / skilled nursing facility setting
- Administer medications and treatments in accordance with physician orders and facility protocols
- Monitor and document patient conditions, progress, and responses to care
- Collaborate with interdisciplinary care teams to support resident health and safety
- Maintain accurate records using PointClickCare (PCC) electronic health record system
Required Qualifications
- Active and unrestricted LPN or LVN license in the state of Vermont
- PointClickCare (PCC) training required
- Must have completed the original or updated COVID-19 vaccine series, or be able to present a documented religious and/or medical exemption from employer
- Must present a valid state or federal issued photo ID and wear a photo badge at all times while on assignment
- Local candidates only
Preferred Experience / Skills
- Prior experience in a long-term care or skilled nursing facility environment
- Familiarity with state and federal compliance standards in LTC/SNF settings
