Key Responsibilities

• Conduct skilled home health visits, with an expected 6–8 visits per day across patient populations including cardiopulmonary, geriatrics, neuro, ortho, and oncology
• Perform comprehensive patient assessments including cardiopulmonary/circulatory, neurological, and orthopedic evaluations
• Develop and implement Home Exercise Programs and provide patient and family education
• Utilize and instruct patients on equipment and devices such as crutches, gait belts, walkers, sliding boards, soft/hard collars, and supplemental oxygen delivery systems
• Complete accurate and timely documentation using designated scheduling and timesheet systems; utilize a provided laptop, MiFi, and portable printer in the field
• Recognize and report signs and symptoms of physical, sexual, and psychological abuse; apply safety and injury prevention principles throughout care delivery

Required Qualifications

• Active Florida or Compact Physical Therapist license required at time of submission
• BLS certification required
• Minimum 2 years of physical therapy experience required
• Home health experience required
• Valid driver’s license and personal vehicle required to perform home visits
• Must be COVID-19 vaccinated or have an exemption form on file; flu shot or declination required — mandatory mask wearing applies in either case
• Locals are welcome — candidates residing within 100 miles of the facility may be eligible; please refer to job title for local acceptance

Preferred Experience / Skills

• OASIS training preferred
• Cerner EMR experience preferred
• Experience working with geriatric, oncology, or complex cardiopulmonary patient populations
