Outpatient Hospice / Medical Social Worker – Days

Key Responsibilities
- Provide medical social services to hospice patients and their families throughout the final stages of illness across a multi-county service area
- Conduct home visits to patients residing in private residences, skilled nursing facilities, assisted living facilities, and adult foster homes
- Deliver counseling, emotional support, and care coordination in alignment with each patient’s hospice plan of care
- Collaborate with an interdisciplinary hospice team including nursing, spiritual support, hospice aides, and volunteers to ensure comprehensive patient-centered care
- Support bereavement services for families following a patient’s passing
- Maintain accurate and timely clinical documentation in accordance with hospice regulations and organizational standards
Required Qualifications
- Oregon LMSW (Masters-level Social Worker) or CSWA (Clinical Social Work Associate) licensure required
- Healthcare Provider Level BLS certification required
- Valid driver’s license and personal auto liability/property damage insurance as required by law; personal vehicle must be maintained in good working order for use in the field
Preferred Experience / Skills
- Oregon LCSW (Licensed Clinical Social Worker) licensure
- Previous hospice experience strongly preferred — candidates with this background will be given preference
- Experience as a social worker in a hospital or medical setting
