Contract | Night Shift | 40 Hours per Week Guaranteed
Key Responsibilities
- Provide direct patient care to residents in a long-term care or skilled nursing facility setting
- Administer medications and treatments in accordance with physician orders and facility protocols
- Monitor and document patient conditions, reporting changes to supervising nursing staff
- Assist with care planning and coordination for residents requiring ongoing skilled nursing support
- Maintain accurate and timely clinical documentation in compliance with regulatory standards
Required Qualifications
- Active and unrestricted LPN license in the state of Vermont
- Valid state or federally issued photo identification required for facility access
- Must wear a photo badge at all times while on assignment
- COVID-19 vaccination required, or documented religious/medical exemption from employer on file
- Ability to work night shifts in a long-term care or skilled nursing environment
Preferred Experience / Skills
- Previous experience in a long-term care or skilled nursing facility setting
- Familiarity with electronic health records and clinical documentation systems
