Key Responsibilities
- Assess patient conditions and implement individualized care plans in accordance with organizational philosophy, policy, and standards of community health practice
- Provide ongoing education and support to patients and their families throughout the course of care
- Perform admission processes and fulfill case management responsibilities in compliance with applicable policies and standards of practice
- Maintain thorough and timely electronic documentation and facilitate interdisciplinary communications
- Travel to patient cases as assigned across the service area
- Participate in weekend rotations, expected approximately once every six weeks
Required Qualifications
- Active Registered Nurse (RN) license
- Minimum 1 year of experience within a Certified Home Health Agency — experience in hospice, private care, or case management outside of a certified home health agency does not satisfy this requirement
- Minimum 1 year of Case Management experience
- Ability to travel to patient cases as assigned
Preferred Experience / Skills
- Familiarity with electronic health record (EHR) systems and electronic documentation workflows
- Strong interdisciplinary communication and collaboration skills
- Experience managing a diverse patient caseload in a community or home-based setting
