Key Responsibilities
- Provide direct nursing care to residents in a long-term care / skilled nursing facility setting
- Administer medications and treatments in accordance with physician orders and facility protocols
- Monitor and document resident health status, reporting changes to appropriate clinical staff
- Collaborate with interdisciplinary care teams to support individualized resident care plans
- Maintain valid state LPN/LVN licensure and comply with all facility credentialing requirements throughout the assignment
Required Qualifications
- Active LPN or LVN license valid in the state of New Hampshire
- Must present a valid state or federal issued photo ID upon arrival at the facility and wear a photo badge at all times while on site
- Must provide a negative COVID-19 test result (sample taken within one week prior to start date) or proof of full COVID-19 vaccination before beginning the assignment — results must be in hand prior to the start date
- Must comply with the facility’s ongoing COVID-19 testing protocols once the assignment has begun
Preferred Experience / Skills
- Prior experience in a long-term care or skilled nursing facility environment
- Familiarity with mid-shift workflows and staffing expectations in a high-census SNF setting
