Key Responsibilities
- Provide, teach, supervise, and document nursing care for patients in the home setting
- Intervene to promote, maintain, or restore health; prevent illness; and support rehabilitation
- Collaborate with healthcare providers, professionals, and community representatives to coordinate home-based care
- Evaluate individual and family responses to nursing interventions and adjust care plans accordingly
- Function independently under general guidance, applying time management, communication, planning, delegating, and problem-solving skills
- Maintain emotional stability in emergency settings and exercise independent clinical judgment; accept on-call responsibilities as needed
Required Qualifications
- Active Massachusetts RN License (must be provided at time of submission)
- Active American Heart Association (AHA) BLS Certification
- Certified Home Health certification with demonstrated proficiency in OASIS (required)
- Minimum one year of acute nursing care experience; hospital-based experience preferred
- Experience in home health (required)
- Valid driver’s license and personal auto insurance; personal vehicle required for this role
- Must reside at least 50 miles from the facility to be eligible
Preferred Experience / Skills
- Infusion Therapy Certification
- Experience with self-scheduling of caseload
- Working knowledge of home care reimbursement and regulatory guidelines
- Bachelor of Science in Nursing (BSN) highly preferred
