Key Responsibilities
- Evaluate and treat patients within a veterans care and skilled nursing facility setting
- Develop and implement individualized physical therapy treatment plans
- Monitor and document patient progress toward established goals
- Collaborate with interdisciplinary care teams to coordinate patient care
- Maintain accurate and timely clinical documentation in accordance with facility standards
Required Qualifications
- Active Physical Therapist license in the state of Oklahoma (or eligibility to obtain)
- Graduate of an accredited Physical Therapy program
- Ability to work full-time, Monday through Friday with no weekend rotation
Preferred Experience / Skills
- Prior experience in a skilled nursing facility (SNF) setting
- Experience working with veterans or in a VA-affiliated care environment
- Familiarity with geriatric or long-term care patient populations
