Key Responsibilities

• Provide compassionate, culturally sensitive, holistic nursing care to patients in a clinical setting, utilizing the nursing process in collaboration with interdisciplinary team members
• Deliver diabetes education to patients and families living with diabetes mellitus in collaboration with a multidisciplinary diabetes management team
• Administer prescribed medications and treatments in accordance with approved nursing techniques, policies, procedures, standards of care, and applicable state laws
• Develop, initiate, and execute individualized patient plans of care, including patient and family instruction, as prescribed by the physician and/or clinic policy
• Serve as a resource to staff on patient education program and materials development, and act as a patient advocate addressing clinical, psychosocial, and financial issues
• Participate in department performance improvement activities and utilize critical thinking to support patient safety, patient experience, and appropriate escalation

Required Qualifications

• Graduate of an accredited nursing program (education verification required)
• Active RN licensure in the state of Illinois and Wisconsin
• CDCES (Certified Diabetes Care and Education Specialist) certification required
• Approved CPR certification required within 90 days of start

Preferred Experience / Skills

• BSN preferred
• Experience working within a multidisciplinary or collaborative care team environment
• Background in patient education program development and implementation
