Key Responsibilities
- Evaluate and treat skilled nursing facility patients to improve mobility, strength, and overall functional independence
- Develop and implement individualized treatment plans tailored to each patient’s goals
- Collaborate with an interdisciplinary care team to coordinate patient care
- Complete documentation in Net Health in accordance with facility standards
- May be required to float to a sister facility within walking distance
Required Qualifications
- Active California Physical Therapist (PT) License
- Strong documentation and time-management skills
- Flexible and collaborative mindset
Preferred Experience / Skills
- SNF experience preferred; mentorship may be available for candidates with 0–1 years of experience
- Familiarity with Net Health documentation platform
