Key Responsibilities

• Provide direct patient care in a long-term care / skilled nursing facility setting
• Administer medications and treatments in accordance with physician orders and facility protocols
• Monitor and document patient conditions, progress, and responses to care
• Collaborate with interdisciplinary care teams to support patient health and safety
• Maintain valid state LPN/LVN licensure and adhere to all facility compliance requirements

Required Qualifications

• Active and unrestricted LPN or LVN state license
• COVID-19 vaccination (original or updated series) or documented religious/medical exemption from employer required
• Must present a valid state or federal issued photo ID at the facility and wear a photo badge at all times
• Local candidates only — must be a local candidate to be eligible for this assignment

Preferred Experience / Skills

• Prior experience in a long-term care or skilled nursing facility environment
• Familiarity with registry or agency staffing assignments and protocols
